Referral form for Frenum Evaluation

Anticipated prerequisites prior to surgical treatment:
Ability to move the tongue (mostly) independent of the jaw, ability to suction and hold for minimum of one minute (five times), dissociation of tongue control from neck or other muscular compensations, concept that tip of tongue should be on the spot and center on the roof of the mouth, myofunctional therapy session scheduled 48 hours post operatively
